What a tummy tuck actually does

Abdominoplasty removes surplus skin from the lower abdomen and, in most cases, repairs the two vertical muscle bands that separate during pregnancy or significant weight change. That muscle repair is the part people underestimate: it is why the operation can flatten a stomach that no amount of training will flatten, and also why the recovery is longer than patients expect.

It is a reconstructive operation performed for cosmetic reasons. The skin is lifted, the muscle sheath is tightened with permanent sutures, the navel is repositioned, and the excess skin is removed. The resulting scar sits low, usually within the line of underwear, and it is permanent.

Who it suits

The best results come from people whose weight has been stable for at least six months and who are not planning further pregnancies. Both conditions exist for the same reason: the operation fixes the abdominal wall in a particular position, and later weight gain or pregnancy will stretch it again.

• Loose abdominal skin that does not retract, typically after pregnancy or weight loss
• Separated abdominal muscles (rectus diastasis) that cause a persistent bulge
• A stable weight, held for six months or more
• A non-smoker, or someone able to stop several weeks before and after surgery

Recovery, realistically

The first week is the hardest. Most patients stand slightly bent forward because the abdominal wall is under tension, and sleeping is easier in a reclined position. Drains are commonly used for the first few days, and a compression garment is worn for several weeks.

Desk work is usually possible somewhere in the second to third week. Anything involving lifting, core loading or impact waits considerably longer, and the muscle repair is the reason: sutures need time before the abdominal wall can take load again. Your surgeon will give you the timeline for your specific repair, and it is not a timeline worth shortening.

Swelling settles over months, not weeks. The shape at six weeks is not the final shape.

Risks you should weigh

Abdominoplasty is a major operation and carries the risks that come with that. Seroma, a collection of fluid under the skin, is among the more common complications and may need draining in clinic. Wound healing problems are more likely in smokers and in people with diabetes.

Because the operation involves a long period of reduced mobility, blood clots are a specific concern, and this concern is greater when surgery is combined with a flight. Discuss your travel dates with the surgical team before booking anything, and do not book a return flight for a few days after the operation.

Sensation over the lower abdomen is usually altered afterwards, often permanently. The scar is permanent, and while it fades, it does not disappear.
