Usually not a cosmetic decision
Reduction mammoplasty removes breast tissue, fat and skin, and lifts what remains. Most women who ask for it are not asking to look different. They are asking to stop the neck, shoulder and back pain, the grooving from bra straps, the rashes underneath, and the exclusion from exercise that disproportionate breast weight causes.
It is one of the few operations in aesthetic surgery with consistently high reported satisfaction, and the reason is straightforward: it solves a physical problem that has a physical cause.
What the operation involves
Tissue is removed, the nipple is moved to a higher position while keeping its blood supply, and the remaining breast is reshaped. The scar pattern is similar to a lift and depends on how much is being removed: typically around the areola and vertically down to the fold, often with a horizontal component.
The amount removed is agreed in advance in general terms, but the surgeon makes final judgements during the operation based on shape and symmetry rather than hitting a number.
Recovery
Most people describe the immediate relief from the weight as the most striking part. Physical recovery involves several weeks of restricted lifting and a supportive bra worn continuously at first.
Swelling settles over months and the final shape takes time. Scars mature over a year or more and their appearance varies with skin type.
Risks
Changes in nipple sensation are common and can be permanent. Breastfeeding may be affected, and if future breastfeeding matters to you, say so before the operation is planned, because it influences technique.
Wound healing at the junction of the scars is the most common local complication, and it is considerably more likely in smokers. Asymmetry between the two sides can persist, since they rarely start identical.
When this operation is not the answer
- You may want to breastfeed and are not willing to accept that it may not be possible.
- You smoke and cannot stop around the operation.
- Your weight is still changing significantly, which will change the result.
Common questions
Will it help my back pain?
For many patients the load-related neck, shoulder and back symptoms improve substantially. It is not a treatment for back pain from other causes, so an honest assessment separates the two.
Can I choose an exact cup size?
You can agree a direction and a broad target. Cup sizing is not standardised between manufacturers, and the surgeon is shaping a breast rather than filling a labelled volume.
Will I be able to breastfeed afterwards?
It may be possible but it cannot be promised, and some techniques preserve the possibility better than others. Raise it before surgery is planned, not after.
