Hollow Under Eyes: What Causes Them and Which Treatment Actually Fits

Hollow under eyes make a rested face look tired, and they are one of the most commonly mistreated concerns around the eye. The fix depends on what is causing the shadow, so the first job is working out whether you have a hollow, a bag, dark skin or a mix of the three.

The short answer

Hollow under eyes are a groove or shadow between the lower eyelid and the cheek, usually along the tear trough. They are mostly structural: the way the skin is tethered to the bone, plus volume lost from the fat and bone underneath with age. Sleep, water and creams change how the area looks on a given day, but they do not fill a structural groove.

  • A pure hollow with good skin usually responds to hyaluronic acid filler placed deep, near the bone.
  • A hollow sitting under a bag is usually better treated with lower eyelid surgery that repositions the fat into the groove.
  • Fat transfer is the longer-term volume option, with less predictable results.
  • Dark pigment and thin skin are skin problems. Filler does not lighten them.

What under eye hollows actually are

The tear trough is the depression that runs down and out from the inner corner of the eye, over the bony rim of the eye socket. Under the skin, a band of ligament anchors the skin and muscle to the bone along this line. Above it sits the fat that cushions the eye; below it, the fat of the cheek. Where the two meet, the tethered line stays put while the tissue on either side changes, and a shadow forms.

That is why the same person can have under eye hollows at 25 and at 55 for different reasons. In younger faces it is often inherited anatomy: a deep trough, a flatter cheekbone, a prominent eye. Later, the cheek fat thins and descends, the bone of the socket rim recedes, and the fat above the ligament can push forward into a bag, which deepens the groove below it.

What causes hollow under eyes

  • Inherited anatomy. A strongly tethered tear trough ligament or a less projected cheekbone shows up early and runs in families.
  • Ageing. Soft tissue and bone support under the eye reduce over time, and the cheek descends, so the rim of the socket shows.
  • Weight loss. Losing facial fat, from dieting, training or illness, can reveal a groove that fuller cheeks used to hide.
  • Bags above the groove. Fat bulging forward in the lower lid casts a shadow and makes the hollow underneath look deeper.
  • Thin skin and pigment. Thin lower lid skin shows the muscle and vessels beneath it, which reads as a dark hollow even when the volume loss is small.
  • Temporary factors. Poor sleep, dehydration, allergies and rubbing make the area puffier or darker for a while. They exaggerate a hollow; they rarely create one.

One exception matters. If one eye looks sunken on its own, suddenly, or with double vision, pain or a change in eyesight, that is a medical question for a doctor or ophthalmologist, not a cosmetic one.

Hollow under eyes vs bags vs dark circles

Many people searching for a fix for hollow under eyes actually have bags, or a bag with a hollow beneath it. The treatments are almost opposite, so it is worth a simple check in a mirror with light coming from above.

Tear trough filler

Filler for under eye hollows is a soft hyaluronic acid gel placed deep, close to the bone, in small amounts. In the US, the FDA approved a filler specifically for infraorbital hollows in adults over 21 in February 2022, and made a training programme a condition of using it there, which says a lot about how unforgiving this area is.

Good candidates have a well-defined hollow, smooth skin and no significant bag. Poor candidates have very thin or wrinkled skin, large fat pads, or a history of filler making them puffy, because hyaluronic acid holds water and the lower lid shows every excess. The usual side effects are bruising, swelling and lumps. Filler placed too close to the surface can show as a blue tint. The rare but serious risk is filler entering a blood vessel, which around the eye can affect vision. This is why the injector should be a doctor who knows the anatomy and keeps the dissolving enzyme, hyaluronidase, on hand.

Under eye fat transfer

Fat is taken from your own body by gentle liposuction, purified and injected in tiny amounts. It can treat wider hollowing around the eye and upper cheek, and the fat that takes can last. The trade-off is predictability: some of the fat is reabsorbed, swelling lasts longer than after filler, and lumps under thin lower lid skin are harder to correct than with filler, which can be dissolved. It is often combined with lower eyelid surgery rather than done alone.

Lower eyelid surgery with fat repositioning

When a bag sits above the hollow, adding filler underneath only makes the whole area fuller. A lower blepharoplasty deals with both: the bulging fat is released and moved down over the rim into the groove, instead of simply being removed. Taking too much fat out is what hollows a lower lid after surgery, so repositioning is the modern approach. You can read how the operation works on our eyelid surgery page.

The NHS describes the operation as taking about 45 minutes to 2.5 hours, usually as a day case, with around two weeks off work. The complication that matters most for appearance is the lower lid sitting lower than it should. Where the lid is already lax, the surgeon may support the outer corner at the same time, which is the functional form of canthoplasty.

Skin treatments

Retinoid creams, microneedling, laser and injectable skin boosters such as polynucleotides aim at skin quality: thickness, texture and colour. They can make thin, dark lower lid skin look healthier, which softens how a hollow reads. They do not replace lost volume, and none of them should be sold as an alternative to filler for a structural groove.

How to fix under eye hollows without filler

If you would rather not have filler, the honest options are fat transfer, surgery, or improving the skin and lighting the area well. There is no cream, massage or supplement that rebuilds a tear trough.

What home care can do is stop the area looking worse than it is:

  • Sleep and fluids reduce puffiness, which is what makes the groove look deeper in the morning.
  • Daily sunscreen protects thin lower lid skin and limits pigment.
  • Treating allergies stops the rubbing and swelling that darken the area.
  • A light-reflecting concealer applied only in the groove, not over the bag, flattens the shadow.

When treatment is not the answer

  • The shadow is mostly pigment, so volume would not change it.
  • You have large bags, where filler adds fullness to a problem of excess.
  • Your skin is very thin and lax, so filler shows through it.
  • You have dry eye, thyroid eye disease or another eye condition that has not been assessed.
  • You have had lower lid surgery before and the lid position has not been checked.

A good consultation should be willing to tell you to do nothing. The non-surgical facelift guide explains the same principle for the rest of the face.

Treating hollow under eyes in Istanbul

Op. Dr. Arda Akgün, a plastic, reconstructive and aesthetic surgery specialist, performs eyelid surgery and filler treatments at his own clinic in Şişli, Istanbul. His background is here. The first step is the same as anywhere: photos in good light, looking straight ahead and up, so he can tell a hollow from a bag before any treatment is suggested.

Filler is a short appointment, but bruising and swelling around the eyes are common in the first days, and a review a couple of weeks later is part of doing it well. Eyelid surgery needs a longer stay, and you should agree in writing when you can fly and who follows you up once you are home. The NHS guidance on cosmetic surgery abroad is worth reading before you book anything.

Common questions

How do you fix hollow under eyes?

It depends on the cause. A clear hollow with good skin usually responds to hyaluronic acid filler placed deep near the bone. A hollow below a bag is usually better treated with lower eyelid surgery that repositions the fat into the groove. Fat transfer is the longer-term volume option. Thin or dark skin needs skin treatment rather than volume.

Can hollow under eyes be fixed naturally?

Not when the cause is structural. Sleep, hydration, allergy control and sunscreen reduce puffiness and darkness, which makes a hollow look shallower, but no cream, massage or supplement restores the volume or changes the ligament that creates the groove.

What is the difference between hollow under eyes and bags?

A hollow is a groove that stays the same when you look up. A bag is fat bulging forward in the lower lid and gets more obvious when you look up. Many people have both, with the bag casting a shadow that deepens the hollow below it, and the treatments are very different.

How long does tear trough filler last?

Often around a year in this area, sometimes longer. It varies between people and products, and hyaluronic acid filler can be dissolved with hyaluronidase if it is uneven or causes puffiness.

Is under eye filler safe?

In experienced hands it is a common treatment, but the area is unforgiving. Bruising, swelling and lumps are the usual side effects. The rare serious risk is filler entering a blood vessel, which near the eye can affect vision, so it should be done by a doctor who knows the anatomy and can manage complications.

Is fat transfer better than filler under the eyes?

Neither is better in general. Filler is precise, quick and reversible but temporary. Fat can last long term and treats wider volume loss, but survival is less predictable, swelling lasts longer, and lumps are harder to correct. Fat is often used together with lower eyelid surgery.

Why do my under eyes look sunken all of a sudden?

A gradual change is usually ageing, weight loss or tiredness. A sudden change, especially in one eye or with pain, double vision or a change in sight, should be checked by a doctor or ophthalmologist before anything cosmetic is considered.
